General Information

Identification (The unit: Household—Applicant)

1. Social security card
2. Texas driver’s license / Texas picture I.D.
3. Permanent Resident (Green Card)
4. U.S. Passport / Naturalization paper
5. Children under 18 birth certificate.
6. Children (Age 18‑23) must show proof of full time student

Residency (Proof dated within last 60days)

1. One utility bill in the name of the applicant only (Electrical utility, telephone, water, Entex, or cable TV)
2. Current Houston Apartment Association lease agreement signed by the person paying in the rent and listing individual living in the apartment or home; and also indicating the amount of rent charged.
3. Property tax documents (dated within the last 12 months)

Income (Proof dated within the last 30 days)

1. Last four paycheck stubs showing date, name, employer, gross earnings (or two paycheck stubs if paid bi‑weekly)
2. Sworn HCHD wage verification form signed by appropriate employer or representative (HCHD documents are similar to that used by TDHS)
3. Bank statements, investment accounts, any other form of income (i.e., businesses owned, real estate income) to verify not living off savings.

Note:

1. All cards, documents, and letters that show proof of the above must be copied (8 ½ x11).
2. Must make an appointment before you come to Chinese Community Center (9800 Town Park, Houston TX 77036)
3. Please come on appointment day before 9:00am.
